The story of Tender Care Home Health began over 43 years ago. Ann McConnell, a Masters Prepared Registered Nurse, was working in the hospital’s critical care unit when she realized something very important about herself and the career path she had chosen.
Truly, she loved caring for her patients. And more so, she recognized that she was at her very best when her patients were at their worst.
Tender Care Home Health is a home health care agency dedicated to serving our patients with the utmost in advanced medical care. We work alongside the patient, in partnership with our patient’s doctors, to ensure they achieve their healthcare goals. 
We use a unique Enhanced Care Model to implement each patient’s individualized plan of care, allowing them to live fuller, independent lives at home while managing their disease, and preventing further hospitalization.
